In my opinion the Voodoo 3 3000 is best served on a S370 board (Via Apollo Pro 133A chipset) with a Pentium III (800-1000MHz). One good example is the ASUS CUV4X but might be on the high price. Similar boards with that chipset can be found easily and for better prices.

You could also try the old hack of thermal paste with acrylic super glue. Big blob of thermal paste on the center and 4 small blobs of glue on each corner that makes contact with the CPU. It could be a problem of the Ramdac. If you have another ISA card you could try. Usually these Ramdacs even though are different parts, most of the time they are just clones of each other, just look for another chip with "476" on top.

There is a video of PixelPipes on YouTube where he changes a couple of caps on what seems to be the same exact card. So when increased the resolution, and stopping the video just at the right time you can almost see what the value is. I believe is L42 like you said. See attached photo

Check the CPU die, many S462 Athlons/Durons suffer from chipped die. This often occurs when bigger coolers appeared and the weight and dimensions would damage the die during installation process. That is why Intel went with Heatspreader on their P4 line.

Not all TNT2 M64 cards were build identical. There were many vendors, I must've had in my hands over 200 of them (not exaggerating, probably way more). Depending on the model / ram config / cooling / core&memory clock, the performance would vary drastically, maybe even 50% from card to card.
