Glad you figured it out, my next guess might have been a bad rom chip or something else unrelated 😀 At least you did not force flash a soldered ROM chip with wrong BIOS like someone did, I can neither confirm or deny that it was me 😁

Yes if the SCSI adapter has it's own BIOS and allows boot from CD. Some controllers have issues with booting off the NT4 cd... ("The Adaptec 1540 series does not support the bootable format on the NT 4.0 bootable CD. ") Which IBM do you have ? May be easier to just use an IDE and boot from floppy …

Is just just my old eyes or are you using a 25Mhz DX with a 66Mhz crystal? Assuming it is 1/2 like the 16Mhz xtal for 8Mhz ISA bus you should use a 33Mhz cpu.... or maybe am over thinking things 😀

🤣 looks like a cricket or micro mouse parts caused it. Had a mid tower a mouse climbed thru an open ISA slot in back and pee-poo all over the side of the board.... corroded it badly... funny now but not back then...

Hmm reminds me of the special Packard Bell w/Voodoo (yes a MSI board): Ultimate Packard Bell P3 / Voodoo3 Back in the early mid 1990's some boards came with S3 Trio or ATI Rage. Yeah today that may seem standard but back then having it built in back then was actually a bit special.
