I think it's because the game can't open a 640x480x256 screen.
AFAIK, nobody has got this run reliably on an NT OS.
From what it sounds like, yours may be trying to start in VESA mode and might need a VESA tool.
Of course, that's pure guesswork as we have none of system specifications to help troubleshoot.
You can try to download Scitech Display Doctor, and see if it works after installation .... (scitech.com)
