Since Dosbox isn't likely to have changed lately, the fault must be with something you recently did. Probably installed new graphics drivers or something else.
Try different outputs in the dosbox config.

Uninstall DosBox from "Add/Remove Programs" or "Programs and Features" in the Control Panel, download a fresh copy of the latest Windows release from here: http://www.dosbox.com/download.php?main=1 then run the installer exe, and DosBox should be successfully installed with a fresh configuration.

Also, don't forget to check out the FAQ http://www.dosbox.com/wiki/DOSBox_FAQ and the 60 second guide 60 seconds guide to getting your game to run in DOSBox

I don't believe that it will be in the "Add/Remove Programs" as that requires a Registry entry that the installer does not make. DOSBox nor the installer touches the Registry. The only DOSBox reference you are likely to find in the Registry is from Windows itself, i.e. in the MRU cache. You need to use the uninstaller from the Start Menu entry or from the installed folder. Regardless, the uninstaller won't touch the "%LocalAppData%\DOSBox" folder, which would need to be deleted for a completely clean reinstall. The 60 second guide won't help if DOSBox closes on launch before he has a chance to do anything.

@Nic-93, from the Run box (Windows Key+R) type %LocalAppData% then Enter and delete the DOSBox folder. That will force DOSBox to recreate it.

Uninstall it and delete anything remaining in the installed folder. Delete the "%LocalAppData%\DOSBox" folder. Re-download the Win32 installer for DOSBox from:

http://www.dosbox.com/download.php?main=1

Reinstall into an empty folder and start DOSBox from the new shortcuts or from directly double clicking dosbox.exe in the newly installed folder.

If this does not work, there is something wrong with your system, perhaps updated drivers. If so, you could try rolling back the new driver or do a system restore from when it was still working.

Download the installer for the Windows SVN build from http://ykhwong.x-y.net/ and see if that works.

If it doesn't work, you can try running that version with the -noconsole switch, which will hopefully produce a useful error log.
