I have a Toshiba XM-3401B CD-ROM drive which had trouble reading CD-R media. It was very unstable and I would get random read errors. I thought there were a problem with it. The CD test on DUNE CD release was a sure fail.

In my 386 I had a Adaptec AHA-1540B, but upgraded to AHA-1542C. The C version have a BIOS which is incompatible with the Zenith 386-20, as you only get "Timeout failure during scsi inquiry command" on boot. While trying to troubleshoot this problem I was searching around for a new upgraded BIOS for the AHA-1540C/1542C but to no availability. That was untill I went on archive.org and looked on adaptec.com's website from 1997 and noticed there were indeed a BIOS 1.02 (I had 1.01 on my card). I updated and unfortunately it didn't fix the boot issue. I then noticed the text for v1.02 which says "Includes synchronous and fast synchronous support for Toshiba XM-3401B CD-ROM drive.". This did indeed fix the issue on the Toshiba in the machine I had it installed in.

Posting this information and attaching the files/BIOS'es for all 15XX cards as they are quite hard to find.

I've had a problem with a 1542CF in my 386sx computer too, so I swapped it with a 1542B.
mkarcher explained to me that the BIOS tries to use 8-Bit-DMA in the UMB area.

With that old Toshiba drives I also noted problems reading CD-R media.
CD-RW? No way.

Perhaps you try a more modern CDROM.

You don't need a SCSI card with a BIOS for CD-ROM use. That just introduces problems.

I use an AHA-1502 or something in my 386. The most basic 16-bit ISA 50-Pin SCSI card Adaptec made. Along with a Toshiba 4x SCSI Tray Loader.

Zero issues. Reads CD-Rs no problem.
