486DX2-66 was about the borderline.
(Don't know ift that's also implying the 486DX50, though - never had one.)

I once found a Dell with a 486SX-33 (ceramic). No HS or fan; the PSU fan was the system's only fan (and was located all the way on the other side of the case from the CPU socket). It worked just fine.
It won't do any harm to just add the heatsink (you can remove the fan to reduce noise and therefore just have passive cooling).
But, the 486SX-25 never came out with a fan or heatsink and it was also not a requirement by the manufacturer.
