Forgive my stupidity, but how exactly do you use AT Plastic standoffs with an AT mobo?

You have three possibilities
1: You have a hole in your case for a metal screw stand-off like in a modern ATX case. Sometimes there will be a couple of places for these in an AT case (and the other mounting points require plastic standoffs).
There are two plastic/nylon stand-off types
2: You have a larger hole in your case that can accommodate a plastic standoff with a peg on bottom like this one:
It can then be slotted into the motherboard like so:
3: You have a spacer. Same as #2 above, but there is no peg. It just creates a little foot for the mobo to rest against the mounting plate in your case.
It is perfectly normal to use some combination of those three in an AT case.
