Straight in the IDE slot. I have a HTPC case that has a top that is easy to remove...so it's no great hassle to open it up to remove the CF card. I do have a spare 5.25 bay up front so I should probably mount it there one day though.

I mostly use internal adapters and nicely wrap them up with the IDE channel cable as part of cable management. I only use PCI slot cover CFIDE adapters for PCs that don't have another means of transferring files to it (ie. no USB or ethernet).
