Does your setup have the -5v line? The SGBXII/Reveal SC100 is one of the few cards that I have that needs -5v. The shot of the back of your card suggests it does too.

I thought mine was broken with working FM until I added the -5v line to my test setup, then it started working. But I don't recall what I saw when checking whether the card was detected.
