I have found a bug that makes it impossible for DOSBox to use additional MIDI settings from "C:\Users\User\AppData\Local\Temp\DOSBOX.CONF" because you can enter an additional MIDI setting inside the D-Fend Reloaded GUI, but it is not saved inside the file. I used the portable installation of D-Fend Reloaded. For example, if you enter "Teststring" as additional MIDI setting only the line "midiconfig=" is written to the file and not the correct line "midiconfig=Teststring".

Because of this the started DOSBox instance does not use the given setting "Teststring".

Feel free to include a bug fix for this reason.
