Ah, I saw this thread and tried to get it working. Success.
Have you tried low color instead of high color GTA? Low color is 8-bit (256) colors, and high means 24-bit colors. I don't even think vanilla DOSBox can support 24-bit colors, or the 3dfx chip.
Seems like you also need the gtadata folder. I deleted it by mistake and just had gtados, and it suddenly stopped working. If you deleted this folder, then reinstall it, and make sure it's in its proper place.
EDIT: Well, it's so slow for me, it's unplayable. I might tweak some cpu settings.

Well. I have tried the 24-bit version on my Voodoo2 P4 (MS-DOS 7.1). The result is same - DOS/4GW crach. 8-bit or 3dfx versions run fine.
Asus P4P800 SE/Pentium4 3.2E/2 Gb DDR400B,
Radeon HD3850 Agp (Sapphire), Catalyst 14.4 (XpProSp3).
Voodoo2 12 MB SLI, Win2k drivers 1.02.00 (XpProSp3).
I run on a real machine, low color also crashes. P4 @ 2.8 GHz, Geforce 4 MX 420. Reinstall doesn't fix it.
Try running the game without EMM.

This worked surprisingly. Even with NOEMS this happens.
Maybe using different versions of DOS4GW would help, or even replacing it with DOS32A.
Is this an ancient version from an original CD? There might be an official patch out there.
