Wikipedia's AudioPCI page reads,

...FM music was simulated using the Ensoniq wavetable, with the result that most FM soundtracks sounded little more than a jumble of cacophonous notes played on random MIDI instruments....

Maybe that explains

the sound its off..sounds really strange

There's no shame in running DOSBox on Windows 98 if the results are better. It just might not give you that feeling of pride or accomplishment that comes from retro purity (for some people).

If you're using a machine that is capable of running Windows 98, is there really a reason to use DOSBox at all? Wouldn't running DOS natively be better than running DOS through an emulator?

Yes, because the MIDI emulation in DOSBox is superior to the MIDI output of a Sound Blaster Ensoniq, evidently.

Actually, Commander Genius might be the best way to go for Keen 4, but that's as far as it goes.

If I were you, I would spend a few dollars on eBay or at your local Mom & Pop computer shop to acquire an ISA Sound Blaster 16, AWE32 or AWE64. If you get an AWE32 or AWE64 and use AWEUTIL to redirect the MIDI to the wavetable, then the MIDI music in DOS will be glorious!

You could even keep the AudioPCI for cleaner sound in Windows games. If you're using one pair of amplified speakers, you can safely mix the outputs from two sound cards with this home-made device:
http://www.instructables.com/id/Altoids-Tin-18-Stereo-Mixer/

My current retro PC has an AWE64 Value for DOS games & MIDI and a Sound Blaster Live! Value for Windows games. I keep the Live! Value because I hear crackling noises if I play Combat Flight Simulator 2 while using the AWE64 Value. I figure that the ISA bus just can't keep up with the amount of bus data that CFS2 makes. I bought that Sound Blaster Live! Value in 2000 assuming that it would be superior to the ISA sound cards in every way, but it was not. FM music sounds better on the 16, AWE32 and AWE64, and MIDI is better on the AWE32 and AWE64 when the wavetable is used.

Could probably wait a few months from now on a proper SDL Keen 4 port now that the source was just recently discovered and recovered.
