I think for me it would be the 'Covox Speech Thing' parallel audio dongle that came with Leather Goddesses of Phobos (bargain bin special). My favorite game use it with was Line Wars 2. Very atmospheric.
Another contender is a programmable joypad that connects between the AT keyboard & PC.
