I currently experience these graphical glitches with my Voodoo 3 3000 setup, I came to rebuilt after 25 years of storage:

This does only occur with the Quake 3 Arena Demo I have downloaded. Other games like Unreal, Unreal Tournament, Motocross Madness and 3dMark99 rune fine with no such errors. I even got a decent 3dMark score.
The card runs hot and I have now attached a fan. What could be the issue? Is it my driver version (voodoo3-10300.zip) or anything else? Could the card be damaged? Does this early version of Q3A miss Glide support? Where should I start?
From my recollection I had some issues with my Voodoo. I remember graphical glitches when playing GTA3 when it released. I moved to a different graphics card later on. In the mean time I used my Voodoo in a dedicated Linux HTPC as a TV out with no problems. It has been safely stored for the past 15 years.
