Does anybody know what this is?

That would explain the RAM size, tranzistors and jumpers.
I was hoping it was an MIDI device.

Quite hard to find out ... the silkscreen printings turn up no results on google and the 2 larger NEC chips are generic gate arrays allowing no conclusions on their functions.

Might be a midi card ... or a scanner driver ... or a video processor.

Some kind of Apple LocalTalk or serial adapter for PC? Just a guess, since Apple serial/modem ports from that era are basically the same as the port on that card.

It is interesting how the writing on it is completely useless.
Maybe there is some one who knows what an IF2BOARD means?

Probably "Interface 2 Board" 😁

Other hints given:
Apple LocalTalk adapter: Can probably be dismissed as such an adapter is quite a simple device and wouldn't require those bulky gate arrays - also seems to be using Sub-D jacks on PC cards.
Medical device: not remotely, see the botch caps on the solder side, the board you linked is actually TV/broadcast equipment.

Looking at the individual components, JRC 723D is an high precision voltage stabilisator, and Nec uPC844C is an quad speed OPamp. I doubt MIDI needs such things, plus there is no component to galvanically separate the connector from the rest of the electronics, which is usually present in MIDI interfaces.

Can't confirm, but I'm also guessing that this is a scanner card.

MIDI card is pretty unlikely - MIDI uses 5-pin DIN connectors, and it should have at least two of them.

The link is from a medical device part supplier and the name of the board is "IF 2 Board". I of course don't know what on earth "IF 2 board" really means. 🤣

Holy shit i saw that exact video a long time ago, cant believe its the same card 😁
But everything is indicating its a scanner card, so I think thats whats it gonna be in the end.
