I was actually looking for similar things at the time I saw the posts and thought I would save some digging though. These form cards are interesting and I hadn't run into them before.

I am in Italy…

My Asus TXP4 is supposed to use one of these, but I chose to use separate brackets for the USB and PS/2 mouse and wire them into the header. They are both easily purchased on Ebay and the job is pretty simple if you know the pinout for each.

Interesting alternative.
The ATX Form Card has capacitors and ferrite beads which make it more reliable.

I ended up finding some new old stock of this cards and decided to buy two.
Does anyone know if the pinout is the same for all motherboards? I can't understand the pinout in my motherboard manual, few pins match but the rest it doesn't seem to make any sense.

That motherboard looks like my PC Chips M571. From what I understand, those were sometimes rebranded as Amptron/Elpina to avoid the PC Chips name stigma.

If it's the same board, the pinout for the various headers is documented here and also here. It's possible that PC Chips used the same the pinout on some of their other motherboards from that time.

PC#1: Pentium MMX 166 / Soyo SY-5BT / S3 Trio64V+ / Voodoo1 / YMF719 / AWE64 Gold / SC-155
PC#2: AthlonXP 2100+ / ECS K7VTA3 / Voodoo3 / Audigy2 / Vortex2
PC#3: Athlon64 3400+ / Asus K8V-MX / 5900XT / Audigy2
PC#4: i5-3570K / MSI Z77A-G43 / GTX 970 / X-Fi

Yes, my mother board is a PC CHIPS M571 Rev. 3.2A
