I have dual PPro motherboard MS-6103 that has a 5pin header for PS/2 connector right next to the DIN AT keyboard connector. Can anyone recommend a way to add a PS/2 header to it on a ribbon cable so that I can connect PS/2 mouse or keyboard to it. The motherboard documentation doesn't provide any specs around what each pin of the header is. Was there a standard for these type of headers?

Sinclair ZX Spectrum 48k, 48k+, 128k, +2
Amiga 1200, 68030/40mhz
386DX/33, ET4000, SBPro2, MT32
Dual PPro/200, Millennium II, Voodoo 2, AWE32, SC-55
etc.

That PS/2 header is just for the mouse, and for the pinout you'll have to look in the manual, since those headers at the time weren't really standardized

Proud owner of a Shuttle HOT-555A 430VX motherboard and two wonderful retro laptops, namely a Compaq Armada 1700 [nonfunctional] and a HP Omnibook XE3-GC [fully working :p]

With the board removed, use a meter to probe the pins. You will find at least one pin connected to ground, one to a +5V rail. The remaining pins you have to guess at. Try each possible combo until your mouse works. It shouldn't hurt anything if you get the data and clock lines wrong. You'll likely find the extra pin is either ground or not connected.

For ps2 mouse just 4 pins are used:

6-Wire Layout
PIN SIGNAL COLOR
Pin 1 Mouse Data Brown
Pin 2 Reserved Open
Pin 3 Ground Red
Pin 4 +5V Yellow
Pin 5 Mouse Clock Green
Pin 6 Reserved Open

Since you just have 5 pins in one line ignore the pin 6 (that is NC anyway).

I finally got around to determine the PS/2 pinout on this motherboard. Posting it here in case someone is looking for the same info in the future:

Motherboard: MSI MS-6103

PIN 1 is toward the edge of the motherboard, PIN 5 is facing inward.

Sinclair ZX Spectrum 48k, 48k+, 128k, +2
Amiga 1200, 68030/40mhz
386DX/33, ET4000, SBPro2, MT32
Dual PPro/200, Millennium II, Voodoo 2, AWE32, SC-55
etc.
