Try to get a board with the intel 440BX-2 chipset, they seem to be the most compatible ones. The voodoo 5 would go well with a Pentium 3 of around 1GHZ, or even a Pentium 4. Intel boards are very compatible here, and also good quality in my experience. When looking for a motherboard for the voodoo 3, bear in mind that they are only AGP 2X, so check that the AGP slot in your motherboard can fit the card physically.
I think you were spot on; issue was my Voodoo 5 5500 PCI I think.
