I exactly created that and tested on one computer and boot by this disket

in config.sys you should load emm386.exe with some paramethers
DEVICE=C:\DOS\EMM386.EXE RAM I=D000-EFFF FRAME=E000

this should do the trick. There has been a discussion on this some moths ago, it may be helpfull.
