Can anyone identify this mystery video card?

It has a 6845 CRTC on it, so I would assume it’s a CGA clone, but what’s with the two composite jacks?

I can’t remember where I got this. It’s been sitting in my collection for a while.

Googling the numbers on the board returns nothing.

It's similar to this - http://www.uncreativelabs.de/th99/v/U-Z/50364.htm

So, one RCA jack is in color, the other is monochrome.
The jumper may select the font - thick/thin.

Edit: there's Hitachi logo on the PCB...
