1.) Calm down. There's no need to double post. It won't get you more attention.
2.) Unzip the file.
3.) Read the DOSBox readme and the FAQ's you'll find on VOGONS and on various places on the web.
4.) If you have a specific problem, post again with _detailed_ information about your problem, your system, and your DOSBox config.

First you unzip that 4keen.zip into a folder called 4keen.

Next, open the 4keen folder and drag INSTALL.EXE onto the DOSBox icon on your desktop.

The installer will create a new folder, KEEN, inside the 4keen folder.

To run the game, drag the 4keen folder onto the DOSBox icon, and enter these commands in DOSBox:

C:>cd KEEN

C:\KEEN>KEEN4.EXE

I had also downloaded Keen 4 as a zip file, but after reading both the Wiki on DOSbox, and your posts, I have worked out how to install and run Commander Keen 4 on my computer (a Vista too 😜).

After dragging INSTALL.EXE onto the DOSbox icon, I realised that I hadn't dragged the other two files (one being ck4sw14.shr) into the file I had dragged install into, but once I did it actually installed Keen 4. Can't wait to play that old favourite 😁
