Kindly do some research on the topic before posting things like that. This card was released under a bunch of different names.

My particular model was called Sound Blaster PCI 128. I know that because I bought it personally back in 2001. It's also what is written on the driver CD that came with my card, and in the PDF manual which shipped on that disc.

PC#1: Pentium MMX 166 / Soyo SY-5BT / S3 Trio64V+ / Voodoo1 / YMF719 / AWE64 Gold / SC-155

The Ct4810 is actually Sb Vibra128 Pci, a variant of the Sb128 Pci

Please re-read my post carefully and click on the supplied link to the driver CD. It has a picture of the disc and the contents of the DISK.ID file.
