Hi, it's just an idea but did you try accessing it from MS-DOS 2.11 too?
Because, MS-DOS 2.11 is from 1983 and can't handle newer floppy drives without help.
Which in turn means it assumes older floppy capacities from the time.
So if PCem (for whatever reason) reports a "wrong" drive/drive geometry, MS-DOS 2.11 won't use it but sticks to its period-correct defaults.
Could be useful for this 8 sectors per track vs 9 sectors per track difference.

Under PCEM - it's a floppy image?
How did you make it?
What exactly is the image size?
If you have the original diskette, I stringly recomend making an image of it using my
ImageDisk (available on "Daves Old Computers") - this will perform an analysis and
determine exactly what the format of the duiskette is!
