Looks like it has a Yamaha YM3812, and the white volume knob reminds me of the old MEDIA VISION THUNDER BOARD.

Has only IN and OUT plus, what looks like a GAMEPORT.

Also, in one of the photos, it looks like it has some sort of ROM with a white label, similar to the MEDIA VISION THUNDER AND LIGHTNING board, but that has VGA incorporated.

This is in a very old 386 system, and it is tucked away in storage, so I can't get any more photos.

The hunt is on, people.

Wow, nice job! That's a pretty obscure one.

I read the OP and hoped that no one had identified it yet. I love trying to ID obscure old parts that are barely visible like this. 😁

Since I got it right, I figured I can ask for another sound card ID in return?

Picked this one up recently for the CD interfaces, but it's quite interesting. Not only does it have separate headers for each interface, it has the OPTi MAD16 chipset, a Crystal chip, Yamaha YMF262...and an Ethernet (or is it SPDIF) port?

Made in Canada it seems.

What is this? It's also a very long boy...SB16 included for size reference.
