Recapped a mac edition of voodoo 5 that has the dvi port. Vga works, dvi outputs patterned noise. No obvious solder or missing component issues on the board.
Attached an example of the output. Any guidance on where to start?

This could be tough to solve, not many people are using DVI on Voodoo cards. Recently I briefly tested V5 MAC and it worked fine.
Does the broken output occur also in BIOS before driver loads? What PC do you run the V5 in?

It’s in the natural state, on a mac G4 MDD. It’s like this from power-on, well before driver is loaded.
I could try flashing it to PC FW and use in my PII rig if this might help in troubleshooting.
Oh, interesting - using Voodoo5 MAC in actual MAC. 😀 I think you are the only one here. Unfortunately I have zero experience with MAC.
Flashing PC BIOS would make troubleshooting easier I guess.
But one more question first - what monitor/screen are you using? V5 DVI implementation is rather old and may have difficulties with EDID on new monitors. For example - I tried V5 MAC and V4 AGP with DVI on Samsung 226BW. No problems there (other than lack of support for native 1680x1050 resolution in drivers).

Tested on a Dell 2208wfp, 2008 era. I can try with another display and edid cloner i have, but I’ve generally had excellent vintage compatibility with this display.
