Using D-fend. Playing Betrayal at Krondor, FYI.
But:
I must be doing something wrong in the settings department but I can't figure out what; no matter how I edit the profile (the config file of Dosbox)of a game, the window stays tiny (about 3x2 inches) and in full screen mode not much more of the screen is used (about 5x2.5 inches).

What am I doing wrong??

1) You are using D-fend. If you really want to use it, better upgrade to D-fend Reloaded.
2) What is fullresolution=...... ?
3) What is output=.... ?

Try this:
fullresolution=0x0 # or your display's native resolution
output=ddraw # or opengl or openglnb or overlay
aspect=true
