This is either glaringly obvious and I've forgotten or there's no easy answer...

Anyone know why certain DOS titles that use MIDI have notes that sometimes become "stuck"? IOW, they should cut off or fade away, but never do...until another note is played on that same channel (causing a slightly different stuck note).

Better still, does anyone have a solution?

Most recently experienced this with "Xian Jian Qi Xia Zhuan".

Happens when the game fails to send the note-off event(s) or screws it up and sends a foobar combination of events instead. VDMSound logs screwed up events in VDMS.LOG. Or the game tries to imitate note-off by pan or volume settings, which different MIDI hardware may interpret differently, or if the game expected MT-32 then maybe the note turned off automatically on the MT-32 because of its limited polyphony (when the partials reserve was exhausted) so they never bothered with ceratin note-off's. Dunno.

So that's a "no easy answer". Was afraid of that...
