I currently have this card with a DB60XG attached, and I am interested in getting an MT-32 external box. Can I have both connected at the same time? According to the manual, the audio out connectors on the MPU-401AT only function when a DB is attached.

MT-32 external box has got its own audio out.

AFAIK , yes, both synthesizers will play simultaneously and you just choose what you want to hear since they have different outputs 😀
