Yes, ATI did I believe.
ASUS did this thing for their MediaBus slots (PCI+ISA combo)
Orchid had something similar. Though it didn't really feature a full soundcard. It just did "voice notes"

The ATI VGA Stereo/FX is the only combo card I know and owned at some point... you also needed it's special cable to actually get the joystick/sound outputs .

That is more or less what I would need, but it is too long for my case.
Can't imagine it is easy to find, either.
Two others worth mentioning are Cardinal Technologies' "SOUNDvision," and Media Vision's "Thunder & Lightning," though neither meets the length requirement.
