Check if the Card has ISA B5 finger with a trace connected to it. Or check if your PSU's have the -5V rail.
Using the correct drivers is important I've found - what's the model number of your card? Both of mine are the studio model, and using the non-studio drivers caused problems - including the installer to report IRQ conflicts that didn't exist.

I know you closed the case, but my sound PAS 16 LMSI worked perfectly in a Pentium board and failed in a 486 machine.
😠 😠 😠 (of course destiny would make me put that sound card in the 486 first)
I am investigating the problem.
About your sound card... It Happens to have extremely low volume for digital effects even with volume maxed out and heaps of artefacts in FM music and low volume either ?
Did you hear the telephone ringing at the screen test ?

I found my PAS Plus kicks off when used with certain PCI graphics cards in the machine. Cards with the TSeng ViPer seem to be a guaranteed problem, the FM music will invariably fail to work.
I tried it in three different ISA machines: an IBM PS/ValuePoint 433DX/Dp, an IBM PS/ValuePoint 425SX/Si and a Magnavox 386SX. No sound whatsoever and I was not even able to initiate it. Are you the guy in PT or CA? I mailed one to CA a few weeks / months ago and one to PT recently. 🤣
I am done with Media Vision: 2 strikes out! With over 50+ Roland PCBs I only had 1 issue once: mono on a RAP-10. 😀 With 40+ Creative cards I only had a SB32 not work once. 😈

Not really.
I am from São Paulo/Brazil. I bet you know some conrades.
If we all say these cards from Media Vision are full of incompatibilities, everyone has to use a Sound Blaster.
It is what they say about SB. The competition is better in paper, but in the end you just use a SB card.
That said, my PAS16 LMSI is doing... pretty well. Lets take a look at the summary so far (PAS16 + Pentium board):
Talking about goals. It seems when you buy hardware, you have a job for the lifetime. So this is where we stand. 😵
So i think my PAS16 can be arranged to perfection. Groove ! 😈 😁 😎
I'm the guy from PT. I have done a quick test for basic functionality. I still need to go deeper and test the inputs and the gameport but for now FM and Speech seem to be working ok.
