I just noticed someone having success with a similar question for Dragon's Lair, so here goes:

I purchased the Action Sixteen CD release of Elite Plus a while ago, but the code sheet was not included. I think this was a jewel-case sized red paper insert with blue or grey text to make it hard to copy. I would like to recreate it (a placeholder until I find a real one, which has proven to be harder than expected), so if someone has a scan, picture, or text file with the contents, basically anything that could help, that would be very welcome.

As far as I know the game has been released with the copy protection removed a long time ago, so I think this should be ok, but if not let me know. Would be happy to prove ownership if required.
