I found this card in my IBM 5150. It appears to be a AST Six Pak Plus clone of some sort (same functionality) but I have no idea what brand it is or where I can find the manual for the dipswitch settings. I think it has game port functionality through an external plug, but I'm not sure if it will work without the right switches. Any help would be nice.

Sorry, can't really help. 🙁

Just a few thoughts.: The left dip switch is for RAM setup, the right one seems to be for serial/parallel.
Drivers for the clock could be generic, so just search for the clock name (or try any XT clock driver).
Also, MS-/PC-DOS 3.30 and up support the AT clock, so maybe no additional driver is necessary if it is compatible.
The two large 40pin(?) chips could be UART chips for serial ports.
