Anyone have any suggestions for new cases, used cases, new-old-stock cases?
-3.5" external bay (for floppy)
-front USB 2.0
-fit a full-length ISA card (about 13 inches / 330 mm)
-80 or 120mm fan(s)
It's not easy to find a case that will fit a full length ISA card anymore.
I use Fractal Design R5 cases for almost all of my retro systems. There's plenty of space, I can even fit my Voodoo5 6000 in there, mounting spaces for huge fans everywhere (140mm), noise absorbing material pre-installed, dust-filters included, not too expensive... Overall, a very nice case, and available in black as well as white. You'll only be needing a 5.25" to 3.5" adapter because it only has two 5.25" external drive bays.

Antec VSK400E. Best deal I've found was on ebay for around 33 bucks. Has (3) 5.25 and (1) 3.5 external bays plus numerous spots for fans.
