Does anyone know this sound card?

The attachment IMG_20171227_201949647.jpg is no longer available

Has an OPTi 82C924 and AD1845 SoundPort, so nothing special. Works fine with the MAD16 drivers. However, I'm interested in the manufacturer of the card. Seems like there was a better version with on-board wavetable as well.

There's no FCC ID or any other useful number on the card.

No idea about the brand/model of card, but I suspect the LS-212 chip is a knockoff OPL3.

Yes, I know. This is the third card in my collection with a LS-212 chip. 😀 Those are real but unlicenced OPL3s in disguise.
