Here is a card that I received today with a lot of other cards.

I am unable to find any information on this card except that is was made by Viacon:

It does have OPL2, and adjustment for IRQ and which CD-ROM interface to use as well as switch labeled G I MIDI.

It also looks like you can route the PC Speaker in as well as maybe sound out to the PC Speaker.

Apparently Goldstar made a 16-bit card that uses the same main chips but not finding the drivers for that card either.

Edit: Is it the Gallant SC-4000?
If so, the drivers are on vogonsdrivers:

Doey it really need drivers ? looks like you simply set IRQ and Adress with DIP Switch.
and just SET BLASTER=mysettings...
