Interesting beast!

It's a Hercules card, and if that's an Intel 82706 (can't make it out on the picture), VGA. The right-hand side of the card is pretty standard, the left less so. That 50p connector at the top looks SCSI-ish, but the socketed chip is a TMS34010 i.e. TIGA, which would suggest SVGA accelerator. And apparently it takes 30p SIMMs.

There was supposedly Hercules TIGA cards called "Graphics Station" and "Chrome", so I guess this could be one of those.

WinXP : PIII 1.4GHz, 512MB RAM, 73GB SCSI HDD, Matrox Parhelia, SB Audigy 2.
Win98se : K6-3+ 500MHz, 256MB RAM, 80GB HDD, Matrox Millennium G400 MAX, Voodoo 2, SW1000XG.
DOS6.22 : Intel DX4, 64MB RAM, 1.6GB HDD, Diamond Stealth64 DRAM, GUS 1MB, SB16.

Wow, a card with DIPP, SIPP and SIMM memory...

And I'm apparently not familiar enough with TI boards... what is the other socketed Intel chip?

Now for some blitting from the back buffer.
