The Telepath seems to be an early modem, perhaps with 28800 bps or 32600 bps or so.
The Telepath for Windows X2 seems to be an WinModem with 56k from ca. 1998..
- I've found some information, but it was scattered around.

I found it: is CJE-0478 at the bottom of the FCC list you show and points to the same model 871731 00 as on the white sticker.
Did not help much as they currently do not have any of the older drivers at the USR website 😒
I think you are correct and is a very early WInmodem version....

Strange, as 7502311 definitely seems to be the correct version - only 7502148 seems older (possibly the early shipping version) and I can't find it archived anywhere online

How are you installing - thru the install app or the inf, and are you clearing out files etc between each attempt

Ahh you did not get all the files onto the floppy OR you have something blocking files like *.drv !.
There are 24 files in 7502311, one is that is missing in your picture is Telepath.drv.
So do this: Open a Command Prompt / DOS Window and with the floppy disk in, do a DIR A:
and see if it has all 24 files....if not then recheck where you extracting and make sure there are 24 files. If there is not:
If in Explorer properties you have "hide hidden system files" checked then you will not see files like Telepath.drv and also cannot copy them to floppy....
That is my guess what is up.....

nope, kinda gave up
