Filled in the form. Just found a Contura at work, which is excellent except for having no sound - so need a solution now! This card is amazing, have been following the PicoGus and PicoMem projects, and having this for PCMCIA is awesome.

Those chips are not readily available. There are some Zilog controllers on eBay, and I haven't seen any Yamaha chips recently. Octoparts shows availablility from Chinese resellers tho. Also, I don't have parts to provide kits.
Tho, if you procure the boards and chips, and assemble them, feel free to sell the results.
