I couldn't find the exact same one, but it looks like the Gateway Lawman. The difference is that the Gateway doesn't have the integrated 3D Rage video and Ensoniq sound.

Appear to be a gateway splash motherboard. Available at ebay and walmart as "MB, 4000276, Audio/Video, gateway splash"
