One last idea so far: Both OS/2 Warp and Windows 98SE ship with Pro Audio Spectrum 16 drivers.
So if it's a software issue, then these Protected-Mode operating systems may behave differently.
(Assuming that Pro Studio 16 uses compatible chips and drivers)
Edit: When I had a PAS16 (Pro Audio Spectrum 16) it was installed in an 12 MHz 80286 for most time.
Though I also moved the card to a Pentium 75, eventually and used it under Windows 98SE for a while.
Because it was my CD-ROM controller, too.
Edit: What I mean to say is that it might be related to timing issues or bus issues.
The 80286 PC ran bus at 12 MHz, whereas the Pentium 75 ran it decoubled from PCI bus, at standard speed.
Maybe your VLB mainboard uses some timings that are too fast somehow?
It's just a wild guess, the PAS series is from around 1992 so 486DX-25, 486DX-33 did exist already.
And the 486DX2-66 uses same bus timings as the 486DX-33.
Edit: Another idea, use a newer or different PAS16 driver diskette.
It's just another silly idea, though. The problem probably is something else.
Doesn't hurt to rule out this possibility of a driver issue, though.
