I can't type the underline "_" on the DOSBox keyboard. If I press "shift minus" I get a "?" . Is there a special combination? I'm using a mac.

You forgot to mention that you are using a German keyboard. You need to give the command KEYB DE.

Can I change the preferences so that DOSBox always starts with the german keyboard layout?
Yes. Add a line containing keyb de to the end of your dosbox.conf file, below the [autorun] section.

Or scan your config file ("dosbox.conf", or usually the pre-defined "DOSBox Preferences" file
in the Mac port) for "keyboardlayout":
# keyboardlayout -- Language code of the keyboard layout (or none).
keyboardlayout=gr
gr or de should give the same result. gr is used oftener, I think.

I don't know how it works on a Mac, but a standard install of DOSBox in Windows produces an entry in the Programs menu as shown below. Reading the aptly named README file is done by selecting that instead of DOSBox itself.

The Mac port doesn't come with an installer, but the users have to copy the application onto the hard disk.
In ideal case, they don't forget to copy the Readme file, too...
In better ideal case, they copy the complete content of DOSBox stuff onto their hard disk.
