I'm using Gulikoza's latest build of DOSBox with Glide support and am having a few issues with Glide.
My first is with resolution. No matter what I set the full screen resolution to the game will always default to 640x480 in full screen mode.
Next up is clipping issues. I took a few screen shots that I uploaded for the Tomb Raider Chronicles forum:

I installed the game using DOSBox and I am using the Voodoo Rush patch for Glide support. My Tomb.exe file is 853KB in size. My setup is Vista Business x64 with all the latest patches including SP1 and an ATI 3870 with the 8.3 drivers. I followed a set of instruction that I found from a user named gidierre over at the Tomb Raider Chronicles forums to setup DOSBox with Glide support.
