It only works if I leave full debug on, in whicbh case the sound is a bit stuttery or out of time at soe points. If I leave it on debug errors only, or no debug, then I just get the PC Speaker (the rattly thing stuk in your PC case somewhere).

Sounds like it's not getting time to return from a call of some sort?

Any ideas?

It's monkey island 1 I'm trying to run, and it's a full DOS game.

Try running Monkey Island with the flag for a specific audio type
eg.
monkey a for adlib
monkey r for mt-32

nope - same thing. Works in debug mode, defaults to pc speaker on normal mode.

It sems like its giving up on its initialisation before some calls come back or something - then defualting to speaker.

It runs fine in soundFX, but I really want it working in VDM cos I know it can and theres no nag screen every 3 mins.

Try looking for SCUMMVM on google. Monkey Island 1 shouldn't be a much demanding game, anyway.

Do you have something that allows direct hardware access (motherboard monitors and the such)? GIVEIO.SYS or similar? Search for any files with the string "Ke386" in them. There should only be one or two, and those should both be VGA drivers.
