Load another program that uses MIDI. That's what I used to do on my 486, which had an SB16 with the hanging note bug.

There's an "all notes off" command for this: just set controller 123 (0x7B) to 0. Though I'm not sure if this affects all channels or only the specified channel.
in hex: Bx 7B 00 (where x is the MIDI channel)

gsplay is a handy util for this kind of thing. gsplay gm/gs/mt32 will send a reset message.
