I have a PC with a Sound blaster 16 and Quake is not outputting sound at all apart from the CD Audio
It does say this in the console also: "Could not reset SBS_INIT: SNDDMA_INIT"

Is your sound card plug and play?
Make sure you have initialised your sound card properly via the Creative plug and play manager if you're playing in DOS.
Run the Sound Blaster Diagnose utility and see if you get sound with your current configuration.

Are you running MS-DOS (like 6.22), DOS from within Windows 9x, or MS-DOS mode from Windows 9x?

How did you install the Sound Blaster drivers?

MS-DOS mode, and I found a driver download here on Vogons and ran the install executable.

If sound in w9x works and other DOS games work with your settings of card like base, irq, dma then try other Quake install files.
If other games don't work too, then check your card's settings: base, irq and dma in device manager of win 9x. Maybe there are conflicts with other devices with settings used in DOS. Also take into account that w9x may to use a separate configs for DOS mode, - check "set blaster" for DOS mode.
