Hey guys, I found this in my box 'o parts and can't seem to identify it. Any ideas?

On the front it says Model C13828, but on the back it says A15828. Elsewhere on the card it says Copywrite Creative Technology, 2000.

Part number looks to be CT-5828
I'm still looking.

edit: The date code indicates it's made at the start of 2000. It resembles a TNT2 that brostenen got.

It looks to be a TNT2 pro.
This page http://buckeyeslist.com/posts/316599, then CTRL+F Creative Labs CT5828 TNT2 Pro 16MB PCI w/DVI (P/N 4001050301) -- $15
It's probably a TNT2 Pro, it fits the puzzle pieces I got atm.
That page got the same card you have pictures at the top right.

That's the one. Thanks! I guess I probably wont end up using it, but always good to have an ID.
