Starting with the SB16. Live!/Audigy DOS drivers don't.
Technically, Sound Blaster cards prior to the SB16 do support MIDI over the Gameport in "SB-MIDI" mode. This would allow for connections to/from keyboards, etc, but not usually in games (I'm not personally aware of any). To be clear, this is neither MPU-401 Intelligent nor MPU-401 UART compatible.
This issue can be mitigated somewhat with Soft-MPU, but has limitations.
