Bad caps or sound chip error?

Could it be an addressing error? What do your SETUPSA and Autoexec look like?

This card has no hardware MIDI synthesizer (Only OPL3L), what you get is Microsoft GS thing with WDM driver or Yamaha's softsynth with Yamaha's VXD drivers. Different driver versions have a different sound. You won't get MIDI out of this card from pure DOS, not without a MIDI addon card or an external module connected to driver.

I am unfamiliar with the sound of the game, try something like Duke 3D, then I'll know exactly what is going on.

Here's what the FM intro should sound like: https://www.youtube.com/watch?v=NBZkCJKsFaU

OP appears to have the percussion but no other instruments.
