I have a 486 pc with a FIC 486-PVT motherboard , with a controller card which only supports serial mice, and I was in need to use a USB one. I found some solutions here like PS2toserial which is sold in serdashop and another PS2 isa card from RIO , but price and availability made me look if aliexpress had something and well it did.

you can query " ps2 to serial isa " in Ali's search bar to find it.

PROS:
-price (I got mine for US$ 18 + US$3 for shipping)
-It works
CONS:
-No documentation
-Odd position of the PS/2 connector (WHY??)
-No brackets
-mouse movement is ok but needs a little bit more accuracy/responsiveness (edited)

At first I was using CTmouse and couldn't get it to work , then I disabled the COM ports on the ISA IDE controller via jumpers and well nothing.... But when i bought this pc , there was a Genius mouse driver and it worked in DOS and windows 3.11. (I haven't tested it further)
It does the job but i'm wondering if I can improve accuracy , reading here I learned that maybe I should get COM speed up to 9600, and try microsoft mouse driver to increase polling rate, maybe to 100hz.

**Currently using a USB optical mouse => USBto PS2 adapter => ATEN KVM => 486 PC**
Hope this info is useful because I couldn't find anything about this adapter.

Any tips to increase accuracy? Thanks!

It's a CS64A , I'm using it with other PCs which have PS2 ports and it's flawless.
In this case (using the ps2 to serial adapter) it's totally usable , don't get me wrong , it just needs a little bit more accuracy/responsiveness

OK. I know some Aten KVMs are fine. Possibly the models I mentioned are OK on certain firmware levels also. Anyhow, if it isn't the KVM I wonder if it is something in how this card works that's causing lag. I thought there was some reason that PS/2 had to be natively supported by the motherboard BIOS in order to work. But the fact you were able to get it to work with a standard mouse driver I guess implies I am mistaken.

Have you tried mouse.com from MS Dos 6.22 install disks. I think that works well for me.

Got one myself as well. Good that you didn't buy the PS2toserial from serdashop as I got one of those as well but it didn't work with my KVM's. One of them is a Aten CS1784.

Also found out that a ctmouse was not working. A couple of others were working ok. I believe I'm using the Microsoft mouse driver with it (version 6.24). Mouse movement didn't seem to be a problem for me but that could also be mouse related. I am using a Microsoft PS/2 ball mouse which is not the fastest mouse to begin with but it's fast enough for the 286 which I am using with this adapter.

I created a basic bracket for it and 3D printed it. Just one with a hole in it for the PS/2 cable. In the future I'll probably just desolder the PS/2 connector and move it to the front but don't have the right PS/2 connector atm (they are available pretty cheap on Aliexpress).

Usually Microsoft Mouse 8.20 is the most compatible.

I am aroused about any X86 motherboard that has full functional ISA slot. I think i have problem. Not really into that original (Turbo) XT,286,386 and CGA/EGA stuff. So just a DOS nut.
PS. If I upload RAR, it is a 16-bit DOS RAR Version 2.50.

I was testing againg and the lag/accuracy was due to the kvm. When using it with my computers with ps/2 ports there wasn't any.

Hmm. That is odd. Possibly something with the timing of the port is causing an issue with the KVM
