Never seen a card this modern (1994 bios date) with SLT-style chip packaging, and couldn't find any similar cards online. I guess some AIB manufacturers would use whatever surplus ram they could find?

I don't know what speed the ram is.

Yes, such chips did sometimes appear on Trident cards - "Input not supported" error when resolution set to 800x600 256 color
